How to refer your patients to the service

For the NKCOS we have introduced a Single Point of Access which allows patients and referrers to access the service easily.

Introducing the Single Point of Access for all ophthalmic referrals will mean that patients will receive the right care, in the right place, at the right time, first time.

How do I refer?
All ophthalmic referrals should be sent via NHS e-Referral Service or safe haven fax to our Single Point of Access. GPs and Optometrists can also refer to us directly by clicking on the button below and completing our electronic GSO18 form. NKCOS will triage all referrals and direct patients to the most appropriate service for their needs, offering a choice of location, date and time for their appointment so the service is as convenient as possible for all patients.

However, 2WWs and ophthalmic trauma cases should be sent to hospital via the usual 2WW and emergency pathways

We will be supplying pharmacists and urgent care practitioners with patient self- referral cards that will be completed and given to patients so that they are able to self-refer through our website or by calling the service. GPs should refer patients via directly bookable assessment clinics on NHS e-Referral System.
